This return form is for customers in the United Kingdom. If you’re in the United States, please visit: https://us.urethrotech.com. If you’re based outside the UK or US, please contact your local distributor—they are responsible for managing all returns and replacements in your region. Do not use this form if your delivery address is outside the UK. Returns from other regions will be rejected unless submitted through the correct channel.

Return Requests Must Be Approved

Please use this form to request return of unused and unopened UCD products. Once submitted, our sales or distributor team will review your request. Do not use this form to report faulty or damaged items — please use the Product Feedback Form instead.

Do Not Ship Products Without Authorisation

Submitting this form does not authorise you to send back any product. You must wait for: 1. Confirmation of eligibility, 2. Instructions on how to prepare a packing list (with batch numbers and quantities), 3.The correct warehouse return address, and 4.Courier booking and tracking instructions. Unapproved returns will not be accepted and may delay or invalidate your free-of-charge (FOC) replacement.

To help us process your return efficiently, please describe the reason for your request and upload the following: – A copy of the original Purchase Order – Clear photo(s) of the unopened product box, showing the REF product code on both ends. If returning multiple units, stack the boxes so all sealing labels are clearly visible in a single photo. All returns must be pre-authorised. Maximum 10 units per request unless otherwise agreed in writing.
Please note, opened boxes are not eligible for return.
